Hard-Soft Night Guard Product Page — B2B Professional

1. What Is a Hard-Soft Night Guard and When Should You Prescribe One?

A hard soft night guard—also called a hard soft occlusal guard, a dual-laminate, or a bilaminar night guard—combines two materials. The construction is the same one described as a "night guard soft inside hard outside":

  • Hard outer surface: Rigid acrylic that distributes occlusal forces evenly and resists wear-through
  • Soft inner layer: Resilient ethylene vinyl acetate (EVA) or silicone that cushions the dentition and improves patient tolerance

Clinical Rationale

When dual-laminate fits:

Heavy bruxers who destroy single-layer soft guards in weeks

Patients who rejected a full-hard guard due to mucosal discomfort or gag reflex

Mixed TMD presentations (joint pain + muscle soreness) where you need both force distribution and shock absorption

The Protective Role — Not a Treatment Device

Night guards are protective appliances, not treatment modalities.

Evidence base:

  • Cochrane systematic review (Macedo et al., 2007): "There is not sufficient evidence to state that the occlusal splint is effective for treating sleep bruxism." The reviewers note that splints are usually used to prevent tooth wear, and that some benefit for tooth wear is possible. Cochrane Database Syst Rev 2007, CD005514
  • National Institute of Dental and Craniofacial Research (NIDCR): TMD symptoms often go away on their own, so experts recommend conservative treatment first — self-care, jaw exercises, and over-the-counter pain relievers — and advise avoiding treatments that permanently change the bite or joints. Lab-made night guards appear among the intraoral appliances a dentist may suggest, not as a first-line "cure." NIDCR — TMJ Disorders

Known Risks and How to Mitigate Them

Delamination (layer separation): User forums report bond failure after 3–12 months, often linked to patient cleaning methods (boiling water, UV sterilizers).

Design countermeasures (Istar protocol):

  • Mechanical interlocking + primer-bonded interface during thermal lamination
  • Edge seal inspection (no exposed lamination line where moisture infiltrates)
  • Patient instructions: Rinse with cool/lukewarm water only; no dishwasher, no boiling, no UV exposure

Occlusal changes & open-bite risk: A review in the British Dental Journal (Bereznicki et al., sj.bdj.2018.869) reported cases where night-guard use produced anterior open bite and other occlusal derangement, concluding that such adverse effects "may be found to be more common than anticipated from existing literature." Follow-up is essential regardless of appliance type, and partial-coverage appliances require especially careful monitoring.

Clinical mitigation:

  • Limit wear to sleep hours unless specifically indicated for daytime clenching
  • Schedule 3-month occlusal checks in the first year
  • Document baseline vertical dimension and monitor

Maxillary vs. Mandibular

Default: Maxillary (easier adaptation, less gag trigger). Mandibular: When maxillary retention is poor (short crowns, steep palatal vault, excessive gag) or when bilateral balancing contacts are needed for joint decompression.

Final choice: Follow the prescribing doctor's instruction. When no preference is stated, Istar defaults to maxillary unless case notes indicate otherwise.

2. How We Manufacture Dual-Laminate Guards

Thermoforming Process

Istar uses thermal lamination (vacuum pressure-formed layers), not milling or 3D printing.

Why thermoforming?

  • Each layer conforms precisely to the scanned anatomy
  • Layer thickness is built to the prescription rather than to a fixed one-size sheet
  • Mechanical interlock created during the lamination cycle ensures bond integrity

Manufacturing checkpoints (from Istar QC protocol):

Verify digital occlusal record + intraoral scan alignment

Mount on articulator to confirm jaw relation

Thermoform the hard shell (acrylic), then the soft inner layer (EVA), each to the prescribed thickness

Thermal lamination: Uniform heating, stable vacuum pressure, controlled cooling to bond layers without bubbles or wrinkles

Trim borders 2–3 mm beyond gingival margins (retention without impingement)

Edge finishing + seal inspection (no exposed seam)

Polish + final caliper check at 6+ points against prescription

Inner surface polishing: Minimal adjustment only (preserve intimate fit); excessive polishing compromises retention.

Material Standards & Compliance

Materials:

  • Hard outer: Medical-grade PMMA, evaluated for biocompatibility under ISO 10993-1
  • Soft inner: EVA or silicone elastomer (same biocompatibility + mechanical testing)

Istar compliance:

  • All materials traceable to ISO 10993 certificates
  • CE documentation for materials; custom-made device documentation per MDR Annex XIII for the EU
  • FDA establishment registration and device listing held (documentation on request)

A note on U.S. regulation: prescription night guards are classified by the FDA as "mouthguard, prescription" (product code MQC), and over-the-counter guards as "mouthguard, over-the-counter" (product code OBR). Both are unclassified devices that go through 510(k) premarket notification — 510(k) clearance is not the same as FDA "approval."

Quality checkpoints (3-stage):

Pre-production: Scan integrity + bite record verification

In-process: Layer thickness + bond + articulator check

Final QC (independent inspector): Caliper, edge smoothness, retention test on stone model, visual check for cracks/bubbles/delamination edges

Failed units are remade automatically at no charge before shipment.

Thickness & Occlusal Design

Thickness and hardness follow the prescription. We don't apply a house default — the layering comes from the doctor's Rx, and we confirm anything ambiguous before production. For heavy bruxers our standing approach is to increase the occlusal-surface thickness and keep that surface uniformly flat, so load is spread rather than concentrated.

Occlusal surface:

  • Flat plane (default): Eliminates interferences, allows free excursive movements
  • Canine guidance or group function: Available on request (specify in Rx)

Edge extension: 2–3 mm beyond free gingival margin; undercuts captured where present; final edge polished smooth.

Service Markets & Client Profile

Geographic coverage:

  • Primary: United States, Netherlands, Germany, France, Belgium, Austria, UK, Luxembourg, Switzerland
  • Cumulative partnerships: 50+ labs, 100+ clinics/hospitals, 400+ direct dentist relationships

Client types:

  • Solo practices & small clinics (recommend consolidating cases weekly to optimize shipping cost)
  • Multi-location DSOs (direct delivery to each site supported)
  • Outsourcing labs (white-label cooperation; your branding on packaging available)

Minimum order: Single-case trials are accepted so you can evaluate our work before committing. For ongoing lab-to-lab (white-label) cooperation we look for regular monthly volume — international shipping makes long-term single-case service uneconomic. Clinic-side accounts carry no minimum, though batching cases (for example, weekly) keeps freight cost down.

4. How to Submit a Case

First-Time Trial

New clients:

Digital cases: Email scans + Rx, or upload via your cloud platform (3Shape Communicate, Medit Link, WeTransfer)

Impression cases (analog): Request a prepaid shipping label from your account manager

We confirm receipt within 1 business day, provide a quote, and start design once you approve.

Case Submission Checklist

Required for every night guard:

  • Full-arch scan of the appliance arch
  • Full opposing arch scan
  • Bite registration (digital bite scan preferred; wax bite photo acceptable if clear)
  • Rx form: Patient name/ID, arch selection, material preference, thickness, special instructions

Optional but helpful:

  • Photos of existing wear facets or failed previous guard
  • Notes on patient gag reflex, limited interocclusal space, prior appliance rejection

Inadequate submissions: Scans with saliva, blood, or motion artifacts → We request re-scan before proceeding.

Design Approval & Production Timeline

Design completed: 24–48 hours after case receipt

Preview sent (STL or screenshot) for approval

Your approval or revision notes (2 free revisions included)

Production: begins once you approve

Final QC + photos (if requested): Pre-ship images for your records

Shipment: Express courier (DHL/FedEx/UPS) or consolidated weekly batch

Turnaround: most cases dispatch within a few business days of approval. Two things don't count toward the promised date: waiting on your approval, missing data, or replies; and weekends plus Chinese public holidays, when we don't produce or ship. Complex cases (specific bite registration, several revisions) simply take longer — we'll give you a date when we confirm the case.

Shipping & Logistics

International shipping:

  • Express: DHL/FedEx/UPS, typically 3–5 days in transit, individual tracking
  • Consolidated batch: weekly departure, typically 5–7 days in transit, lower per-case cost (worth it if you ship regularly)
  • Multi-site delivery: Specify different ship-to addresses per case

Packaging:

  • Each case individually bagged with patient ID label
  • High-pressure cleaning pre-packaging
  • Foam-cushioned outer box with case manifest

Quality guarantee:

  • Manufacturing defect (edge defect, delamination, retention failure on model): Automatic remake at no charge before shipment
  • Defective unit received (not a patient fit issue): free remake — send photos plus the original files

Fit issues:

  • Scan error or incomplete bite record: Remake is billable (new case fee)
  • Design misinterpretation of your Rx: Remake is free (our responsibility to clarify ambiguous instructions)

Patient non-tolerance (comfort, gag): Not covered; design adjustment (thinner, shorter borders) at discounted rate for second attempt.

FAQ

Q: Can I specify the hard:soft thickness ratio? A: Yes — the ratio follows your prescription. We don't apply a house default; note your target on the Rx and we'll confirm anything unclear before production.

Q: What if my patient has limited interocclusal space? A: Note it in the Rx. Thin and flat is the usual answer, and if a dual-laminate won't fit the space available, we'll tell you before production and suggest a single-layer alternative.

Q: Can you make a mandibular dual-laminate guard? A: Yes. Specify "mandibular" on the Rx form. Default is maxillary unless noted.

Q: How do I prevent delamination? A: Patient education is critical. Provide these care instructions:

  • Rinse with cool/lukewarm water only (no boiling, no dishwasher, no UV sterilizer)
  • Brush gently with soft toothbrush + non-abrasive soap
  • Store dry in ventilated case (not sealed wet)
  • Avoid hot car or direct sunlight

Our manufacturing (mechanical interlock + edge seal) minimizes risk, but thermal abuse can compromise the bond.

Q: What's the warranty against delamination? A: A manufacturing bond failure under normal use = free remake. Patient-induced thermal damage (boiling water, dishwasher, UV sterilizer) = not covered.

Q: Can I order just the design (STL) without fabrication? A: Yes. We offer CAD-only service. Contact sales for design-only pricing.

Q: Can I request photos before you ship? A: Yes. Note "QC photos requested" in your case submission. We'll send images of the finished appliance on the model before final packaging.
