Product Range
Available configurations, fabricated to prescription:
- Custom mandibular advancement device (MAD) — full-arch, dual-jaw
- Dual-laminate MAD — rigid outer shell with soft EVA inner liner
- Rigid acrylic occlusal splint — milled PMMA, minimal bulk
- Monobloc vs. two-piece designs — upper-only or upper-and-lower splints, upper or lower arch
- Custom mouth guard for snoring patients — same prescription pathway, for primary snoring presentations
- Titrating appliances — incremental mandibular advancement in the field
The terms "custom sleep apnea mouth guard," "custom mouth guard for sleep apnea," and "custom mouth guard for snoring" all refer to this same device category — a prescription mandibular advancement device fabricated from your patient's own scan or impression. What changes between them is the clinical intent behind the prescription and how the appliance is titrated, not the fabrication method.
Upper versus lower arch selection, material hardness, wall thickness, and edge extension are clinical decisions made by the prescribing dentist. We fabricate to your written prescription and will advise on manufacturing options when you contact us with a specific design question.
Clinical Role
AASM/AADSM clinical practice guidelines recommend oral appliance therapy for adults with OSA who are CPAP-intolerant or who prefer an alternative, and for primary snoring. Meta-analysis of randomized trials finds CPAP produces greater AHI reduction in severe OSA while oral appliances perform comparably in mild-to-moderate OSA; objective adherence data show oral appliances are typically worn more consistently per night.
On the fabrication side, fit quality drives outcomes. In a randomized crossover trial, custom-made devices outperformed preformed thermoplastic devices on success rate (60% vs. 31%, P = 0.02) and retention — the same study found the preformed devices unsuitable even as a screening tool. That is the gap this lab works in.
Why Choose Our Lab for Custom Sleep Apnea Mouth Guards?
At Istar Dental Lab, we specialize in the fabrication and manufacture of custom oral appliances for OSA and sleep-disordered breathing—not patient treatment or diagnosis. We serve as a manufacturing partner for dentists, sleep clinics, dental laboratories, and DSOs who need reliable, precision-fit devices for their patients.
Full In-House Production
All devices are produced entirely within our own facilities in Shenzhen and Huizhou, China. We do not outsource cases to third-party labs. This integrated production model allows us to:
- Control every step from digital design to final quality inspection
- Maintain consistent fabrication standards
- Reduce production variability and improve delivery stability
- Provide direct communication between your team and our technicians
Production Capacity and Lead Time Notes
Our in-house technician team (approximately 460 professionals across CAD design, milling, implant, ceramic/metal, removable denture, and quality control departments) supports stable output even during seasonal demand increases.
Lead time starts when we receive complete, production-ready data (scan files or acceptable impressions + prescription + bite registration). Time required for design review, missing data supplementation, or client confirmation is excluded from the committed production timeline.
Working days exclude weekends and Chinese national holidays. For critical deadlines during peak seasons (e.g., November–December), we recommend submitting cases at least 10 working days in advance.
How We Fabricate Custom Mouth Guards for Sleep Apnea
1. Digital Design and Bite Registration
Upon receiving your case, our CAD team:
- Imports scan data (maxillary, mandibular, and bite registration scans)
- Verifies bite relationship using the provided protrusive/therapeutic bite record
- Designs the appliance digitally with controlled mandibular advancement (typically 60–75% of maximum protrusion, adjustable as prescribed)
- Checks for occlusal clearance, undercut management, and retention features
- Sends a 3D design preview for your approval before fabrication (available upon request for complex cases)
Critical requirement: Accurate bite registration is essential. We accept:
- Intraoral scans captured at the prescribed protrusive position
- George Gauge or similar calibrated bite fork records
- PVS/polyether bite registration at target advancement
Do not send "habitual occlusion" or centric relation bites—the appliance must be designed to the therapeutic forward position.
Designing for Side-Effect Mitigation
Mild-to-moderate adverse effects are common with mandibular advancement devices and are influenced by design choices. AADSM's management recommendations for oral appliance therapy identify gingival soreness, TMJ discomfort, excessive salivation, and tooth movement as common complaints, many of which are design-sensitive.
Design parameters we can adjust on your instruction:
Design Element | Adjustment Options | Intended Benefit |
|---|---|---|
Gingival extension | 2–3 mm vs. 4–6 mm coverage | Adequate retention without soft-tissue impingement |
Connector type | Fixed rigid vs. flexible/damped | Reduced TMJ loading during sleep |
Vertical dimension | Minimal opening vs. increased vertical | Spacing to reduce posterior joint compression |
Occlusal coverage | Full-arch vs. partial (anterior) | Posterior disclusion to reduce clenching forces |
Retentive features | Undercut utilization vs. smooth | Retention without lateral stress on teeth |
Finishing polish | High-gloss vs. ultra-smooth intaglio | Reduced tissue irritation and plaque retention |
Most common initial complaints and our responses:
Patient Complaint | Design Cause | Adjustment |
|---|---|---|
TMJ soreness in the morning | Excessive initial advancement | Reduce protrusion by 2–4 mm; if titratable design specified, patient can self-adjust |
Excessive salivation (first 1–2 weeks) | Normal adaptation response; more common with larger vertical opening | Usually resolves; in some cases reduce vertical dimension |
Gum tenderness at vestibule | Overextended gingival margin | Reduce extension to prescribed 2–3 mm guideline |
Tooth movement or looseness | Missing posterior abutment teeth | Consider retentive clasps or immediate denture overlay designs |
All design adjustments should be prescribed by the treating dentist or sleep physician, who evaluates the patient clinically. Our role is to fabricate the device according to your specifications and to advise on design alternatives when you contact us with specific fit or comfort issues.
Note: If your patient develops TMJ symptoms, tooth mobility, or significant occlusal changes during appliance therapy, refer them back for clinical evaluation before continuing treatment or requesting modifications.
2. CAD/CAM Milling or Thermoforming
Depending on the prescribed appliance type and material, we use one of two fabrication methods:
Digital Milling (for rigid acrylic appliances)
- CNC milling from biocompatible PMMA or dental acrylic pucks
- Suitable for hard mandibular advancement devices requiring high dimensional stability
- Allows precise control of wall thickness, connector geometry, and retention features
Vacuum Thermoforming (for dual-laminate and soft-lined appliances)
- Heat-pressing PETG, EVA, or layered thermoplastic sheets over 3D-printed or milled models
- Common for appliances requiring soft inner surfaces for comfort and retention
- Particularly suited for dual-laminate designs (hard outer shell + soft inner lining)
We currently use thermoforming as the primary method for custom sleep apnea mouth guards. Digital milling is available for specific clinical requirements (e.g., ultra-thin posterior clearance, specific connector designs).
3. Retention, Adjustment, and Titration Mechanism
Our mandibular advancement devices incorporate:
- Undercut retention on the lingual and buccal surfaces of posterior teeth
- Precise fit to the gingival margin to resist dislodgment during sleep
- Adjustable connectors or advancement screws (when prescribed) to allow incremental titration of mandibular protrusion
Note: Adjustment features must be specified at the time of ordering. If you require a titratable device (allowing post-delivery adjustment of advancement), indicate this on the prescription and specify the mechanism type (e.g., Herbst-style connectors, elastic bands, fixed connectors with interchangeable lengths).
4. Finishing, Polishing, and Edge Smoothing
All appliances undergo:
- Trimming to prescribed gingival extension (typically 2–3 mm above the gingival margin to balance retention and comfort)
- Hand polishing of all surfaces to remove tool marks, rough edges, and sharp margins
- Magnification inspection for burrs, thermal distortion, or fabrication artifacts
Appliances with rough or protruding edges, inadequate polishing, or sharp connector components will not pass our internal quality control and will be reworked before shipment.
5. Quality Control and Functional Verification
Before an appliance leaves our lab, it must pass:
- Model fit test: The device is seated on the working model and checked for complete seating, absence of rocking, and retention under simulated removal force
- Dimensional verification: Wall thickness, connector clearance, and occlusal relief are spot-checked with calipers or digital measurement
- Visual inspection: No voids, cracks, discoloration, or contamination
- Prescription compliance review: The case coordinator verifies that the fabricated device matches the submitted prescription (material, design features, advancement distance, retention style)
Appliances that fail any checkpoint are either reworked or remade, depending on the nature of the defect.
Material Options and Biocompatibility
We fabricate custom sleep apnea mouth guards using materials selected for prolonged intraoral contact, supplied with biocompatibility documentation from the manufacturer.
Thermoplastic Materials
PETG (Polyethylene Terephthalate Glycol)
- Hard, durable outer shell material
- Resistant to fracture and wear
- Suitable for patients with moderate to heavy bruxism
- Clear or tinted finish available
EVA (Ethylene Vinyl Acetate)
- Soft, flexible inner lining material
- Provides cushioning and enhanced retention
- Commonly used in dual-laminate designs (PETG outer + EVA inner)
- Not recommended as a standalone material for patients with severe clenching
Dual-Laminate Construction
- Combines a rigid outer shell (PETG or similar) with a soft inner layer (EVA)
- Balances durability and patient comfort
- Most commonly prescribed for first-time oral appliance users
Rigid Acrylic (PMMA)
- Heat-cured or milled polymethyl methacrylate
- High dimensional stability and long service life
- Thinner wall thickness possible compared to thermoformed materials
- Preferred for patients requiring minimal bulk or specific occlusal designs
All raw materials come with manufacturer certificates of compliance and material safety data sheets (MSDS/SDS), available upon request.
Material Traceability and Registration
For each fabricated appliance, we maintain records that allow traceability to the material batch, production date, and material supplier. Upon request, we can provide:
- Material Safety Data Sheets (MSDS/SDS) for all polymers and resins used in fabrication
- Certificates of Compliance from material suppliers, including biocompatibility data
- Batch/lot identification for materials used in your specific case
- Manufacturer declarations of conformity for CE-marked raw materials
This documentation is particularly important for practices that treat patients in jurisdictions with strict medical device traceability requirements, or for patients who request material safety information due to sensitivities or allergies.
Registrations and quality system:
Istar holds current FDA registration, EU MDR notification, MHRA registration, and ISO 13485 quality management system certification.
A note on how to read these: FDA establishment registration and device listing, EU MDR Annex XIII custom-device notification, and MHRA registration are administrative market-access registrations. They are not regulatory approval or product certification of any individual device, and none of them means a regulator has reviewed or endorsed this specific appliance. ISO 13485 certifies our quality management system, not the device.
For EU shipments, custom-made devices follow the MDR Annex XIII custom-device pathway: we issue a custom-device statement of conformity and complete the custom-device notification to the competent authority, rather than going through a notified-body conformity assessment. Certificates, registration numbers, and technical documentation are available on request for credentialing or facility audits.
Prescription Requirements and Case Submission
To ensure accurate fabrication, please provide:
Required
- Maxillary and mandibular arch scans or impressions (PVS, polyether, or digital)
- Bite registration at the prescribed protrusive position (not centric occlusion)
- Prescription form specifying:
- Appliance type (MAD, tongue-retaining device, or other)
- Material preference (PETG, dual-laminate, rigid acrylic)
- Mandibular advancement distance (in mm) or percentage of maximum protrusion
- Retention style (full-arch coverage, partial coverage, specific tooth undercuts)

Common Questions About Custom Sleep Apnea Mouth Guards
Do you treat patients directly?
No. We are a dental laboratory and manufacturing facility. We fabricate devices based on prescriptions from licensed dentists, sleep physicians, or other qualified healthcare providers. Diagnosis, prescription, fitting, and follow-up are the responsibility of the prescribing professional.
Can I submit a case without a sleep study?
We require that all custom oral appliances for OSA be prescribed by a qualified professional. While we do not require you to submit a copy of the sleep study report, the prescribing dentist or physician must have diagnosed the patient and determined that oral appliance therapy is clinically appropriate.
According to AASM/AADSM clinical practice guidelines, oral appliances are recommended for adult patients with OSA who are intolerant of CPAP therapy or prefer an alternative treatment, and for patients with primary snoring.
Is oral appliance therapy covered by Medicare or private insurance?
Medicare coverage (United States):
Custom oral appliances for OSA are covered by Medicare under HCPCS code E0486 ("oral device/appliance used to reduce upper airway collapsibility, adjustable or non-adjustable, custom fabricated, includes fitting and adjustment") when:
- The patient has a diagnosis of obstructive sleep apnea confirmed by polysomnography or home sleep apnea testing
- The device is prescribed by a physician (MD/DO) or dentist acting under a physician's order
- The device is custom-fabricated (not an over-the-counter or boil-and-bite appliance)
Refer to CMS Local Coverage Determination L33611 and Article A52512 for full billing and documentation requirements.
Private insurance:
Most major U.S. commercial payers cover custom oral appliances for OSA under durable medical equipment (DME) benefits when medical necessity is documented (sleep study + trial/failure/intolerance of CPAP, or contraindication). Coverage policies vary; verify eligibility with the patient's plan before fabrication.
Important: Billing, coding, and insurance claims are the responsibility of the prescribing provider or the patient. We do not process insurance claims directly.
What if the appliance doesn't fit or the patient cannot tolerate it?
If the appliance does not seat properly on the patient's dentition:
- Verify that the bite registration used during fabrication matches the current intraoral condition (no recent dental work, no tooth movement)
- Check for interferences on the model (e.g., bubbles in the impression, scan artifacts)
- Contact us with photos or a description of the fit issue
Free remake policy: If the fit problem is due to our fabrication error (e.g., incorrect design, distortion during processing, failure to follow prescription), we will remake the appliance at no charge. If the issue is due to inaccurate impressions, incorrect bite registration, or changes in the patient's dentition after case submission, the remake will be charged as a new case.
Can you adjust an existing appliance or add titration hardware?
We can fabricate replacement connectors, elastic bands, or adjustment screws if you provide the original appliance or precise measurements. Important: Modifications to an appliance that has already been delivered and used by a patient may require remaking the device to ensure biocompatibility and structural integrity.
For appliances that require periodic adjustment (e.g., advancing the mandible in 1mm increments over several months), specify "titratable design" on the initial prescription. Retrofitting titration mechanisms to a fixed appliance is not always feasible.
Do you provide patient instructions or home-care guidance?
We can include generic appliance care instructions (cleaning, storage, inspection for wear) with each shipment if requested. However, patient-specific instructions—including wear schedule, adjustment protocol, and follow-up intervals—must be provided by the prescribing dentist or sleep physician.
What certifications and regulatory documents do you maintain?
We hold FDA establishment registration and device listing (for U.S. shipments), EU MDR custom-device notification (Annex XIII), MHRA registration, and ISO 13485 quality management system certification.
Full details — including what documentation we can provide on request (material certificates, MSDS, declarations of conformity, registration numbers) — are covered in the Material Traceability and Registration section above. We also welcome facility visits and video inspections for credentialing purposes.

Request a Quote or Start a Case
For individual dentists and clinics: We accept single-case trial orders. No long-term single-case agreements — international shipping costs make standalone cases uneconomical — but we welcome the opportunity to quote your ongoing volume.
For dental laboratories: For new accounts, we generally require monthly lab fees of $500 USD or above to sustain a cost-effective partnership. Below this threshold, unit economics don't work given cross-border shipping.
Quoting logic:
Pricing is calculated based on:
- Unit count — establishes the base rate
- Material selection — sets the base material price (thermoformed PETG/EVA, dual-laminate, or rigid PMMA)
- Design complexity — advanced connectors, titration mechanisms, and full-arch designs add to the base rate
- Implant components — additional hardware where applicable
All quotes include design, working models, components, individual packaging, and international shipping. There are no hidden fees.
Payment Terms
- Bank transfer and PayPal accepted
- Transaction fees are borne by the customer, settled at the exchange rate on the payment date
