Hard Night Guard

Introduction

Hard night guards (also called hard acrylic night guards or hard occlusal guards) are custom-made oral appliances designed to protect teeth, distribute occlusal forces, and support temporomandibular joint function. They are primarily prescribed for patients with bruxism, clenching habits, or occlusal management needs.

Who Manufactures Your Night Guards

Istar Dental Lab is a full-service dental laboratory and direct manufacturing facility, founded in 2012. We are not a trading company or middleman.

  • Manufacturing facilities: Shenzhen and Huizhou, China
  • Production model: All steps completed in-house—from digital design to final inspection—no outsourcing to third-party labs
  • Team: Approximately 460 professional employees, including CAD designers, production specialists, and quality control staff, plus ~50 trained backup workers for peak demand
  • Customer base: Over 50 dental labs, 100+ clinics, and 400+ dentists in the US, Netherlands, Germany, France, Belgium, Austria, UK, Luxembourg, Switzerland, and other markets

Factory visits welcome: On-site tours, production line walkthroughs, or video inspections can be arranged through our sales team. Customers can verify production workflow, facility environment, and QC procedures directly.

Compliance: FDA-registered (establishment + device listing), EU MDR custom-device notification completed, ISO 13485 quality system active. These registrations are publicly verifiable: FDA establishment and device listings can be searched in the FDA Establishment Registration & Device Listing database. Custom dental prosthetics, including occlusal guards, are covered under our FDA device listings (FDA,MDR,MHRA,ISO 13485 valid; device listings cover custom crowns/bridges/inlays/onlays/veneers/implant abutments/implant superstructures/full & partial dentures/temporaries/custom appliances).

Material and Fabrication Approach

Current production: Thermoformed using heat-activated acrylic sheets ("we currently mainly do thermal-compression molding").

How it works:

  1. Digital design or physical model prepared from impression or intraoral scan
  2. Acrylic sheet uniformly heated, pressed onto the model under controlled pressure ("uniform heating, stable tooth addition, maintain pressure and cool uniformly until hardened before demolding")
  3. Cooling stabilized under sustained pressure to prevent warping
  4. Trimmed, finished edges checked for smoothness and extension
  5. Internal fit evaluated on the stone model for retention and seating ("only minimal grinding, avoid excessive adjustment that causes non-seating")

Hardness and thickness: Determined by the clinician's prescription ("decided by the doctor"). Common ranges follow clinical standards for bruxism appliances; specific durometer/hardness values vary by sheet brand and clinical protocol.

Material selection note: While PMMA (polymethyl methacrylate) is frequently mentioned in online forums and articles, we use heat-press acrylic sheets specified for dental appliance fabrication. If you require material certificates, biocompatibility documentation, or specific technical parameters, request them at case submission—these documents are provided as part of our standard compliance package ("can provide English custom declarations, material certificates, product labels and related materials").

Who Needs a Hard Night Guard?

Primary indications (based on prescribing dentist's clinical assessment):

  • Bruxism and clenching: Chronic grinding or jaw clenching during sleep, leading to tooth wear, fractures, or muscle fatigue (Cleveland Clinic: Mouthguards; NHS: Teeth Grinding)
  • Occlusal force distribution: Cases requiring even load distribution across the arch to reduce localized stress
  • TMJ support: Adjunctive management for temporomandibular joint dysfunction, under direct clinical supervision
  • Protective wear: Prevention of further enamel loss in patients with established wear patterns

Hard vs soft: Hard acrylic guards distribute occlusal force more evenly than soft guards and are generally preferred for severe bruxism or sustained clenching. According to our internal guidelines, hard guards are (suitable for severe occlusal problems, distributing occlusal pressure, relaxing masticatory muscles, protecting teeth and the temporomandibular joint, and temporarily adjusting mandibular position to improve TMD symptoms). Soft guards may be chosen for mild cases or when joint pain is the primary concern. Dual-laminate (soft inner / hard outer) guards combine both properties. Your clinical judgment determines the best fit for each patient.

Maxillary or mandibular: Both arches are viable; choice depends on patient occlusion, retention anatomy, and clinician preference (follow doctor's recommendation). Most cases are maxillary.

Digital Workflow and Case Submission

Accepted formats:

  • Digital: STL files from intraoral scanners (maxillary prep arch, mandibular opposing arch, bite registration scan)
  • Physical: PVS or alginate impressions + bite record ("we can provide shipping labels, or customer ships to our address and provides tracking number")

What we need:

  1. Maxillary or mandibular arch scan / impression (whichever arch receives the appliance)
  2. Opposing arch (full coverage to last molar, mesial to anterior area, including 2–3 adjacent teeth of prep tooth and corresponding gingival soft tissue)
  3. Bite registration or jaw relationship record (bite wax)
  4. Prescription details: thickness preference, edge extension, any occlusal relief or contact guidance

Occlusal space note: If occlusal clearance is inadequate or deep overbite is present, note "occlusal space inadequate" on the case form and specify remaining vertical dimension (write remaining space thickness, prompt for reduction amount or thin material selection). This helps us recommend material selection or design adjustments before fabrication.

Design confirmation: Before fabrication, our design team reviews the digital setup on an articulator (for digital cases) or verifies the bite record against mounted models (for physical cases: verify intraoral scan and bite record, mount on articulator for secondary verification). If discrepancies arise, we contact you for clarification—preventing remakes and reducing turnaround delays.

Quality Control and Remake Policy

Inspection checklist (per case):

  • Thickness: Calipers used at multiple points; measured values compared against prescription (thickness measured with calipers at multiple points, compared against prescription)
  • Edge smoothness: No sharp burrs or protruding margins; extension range and model fit checked; flush margin contact verified (edge check smoothness, extension range and model fit, no burrs or warping)
  • Retention: Test seating and removal on stone model; retention force (grip/suction) evaluated for adequate hold without excessive difficulty (retention tested on stone model for seating and removal, assess grip/suction force)
  • Surface integrity: No cracks, bubbles, warping, or visible defects; final visual inspection before packaging (finished product with no cracks or bubbles, acceptance per customer-confirmed prescription and design)

Internal surface: Only minimal grinding to ensure full seating (only minimal grinding, avoid excessive adjustment that causes non-seating). Over-adjustment of the internal surface is avoided to preserve fit accuracy.

Free remake — lab-side quality issues: If the guard fails to meet your written prescription due to our fabrication error, we remake it at no additional lab fee. Lab-side issues that qualify for free remakes include (fixed prosthetics example adapted for appliances):

  • Guard does not seat properly on the model (non-seating)
  • Edge lifting or warping (edge warping)
  • Incorrect thickness or design not matching the confirmed prescription
  • Obvious aesthetic flaws affecting clinical use (obvious flaws affecting aesthetics)
  • Occlusal contact errors (when bite record was accurate) (occlusal high, adapted)

Return shipping: Customer is responsible for return shipping of the original guard. We cover outbound shipping for the remake (free remake does not include shipping and implant component fees).

What is NOT a free remake:

  • Customer requests a color change, material change, or design revision after approval (customer changes color, changes material)
  • Original impression or scan had voids, distortion, or incomplete anatomy, and customer asked us to "try our best" despite the defect (requested us to try our best with poor impression)
  • Patient's tooth preparation changed after the original case was submitted (remake scenario allows free redo if customer re-preps tooth; non-remake scenario requires new case at standard fee)
  • Clinical adjustment preference (customer wants a different occlusal contact or fit than originally specified)

Remake priority and turnaround: Remake cases are prioritized as rush. Typical remake turnaround is 3–5 business days (remake goods prioritized for rush, turnaround determined by case specifics, typically 3–5 days of normal production time).

Quality issue escalation: If multiple remake attempts still do not resolve the issue, we provide a refund of the lab fee (refund provided if multiple remakes still cannot fix the issue).

Turnaround Time and Shipping

Standard lead time: Turnaround is calculated in business days from receipt of complete case data (business days) and starts when all required files (scans, bite, prescription) are confirmed usable (design approval wait time, missing data, waiting for customer response time are not counted toward promised lab turnaround; turnaround restarts from receipt of complete materials / customer confirmation). For appliances like night guards, typical production time is 5–7 business days once fabrication begins (complex implant or full-arch case fabrication time? 5–7 business days — hard guards are simpler than complex implant cases, so this is the upper-bound reference). Weekends and Chinese public holidays are non-production days and do not count toward delivery commitments.

Digital vs physical cases: Digital cases (STL files) eliminate physical model shipping time, shortening total cycle by 1–2 days (lab production time is identical; digital cases eliminate physical model shipping and customs, overall cycle is shorter; physical impressions require cross-border logistics time for model arrival).

Rush service: Expedited turnaround available upon request (additional fee applies). Contact your account manager at case submission to confirm availability.

Peak season: During high-demand periods, we may publish advance order cutoff dates and adjusted lead times. Our backup workforce (~50 trained personnel) helps maintain capacity, but early communication is recommended for time-sensitive cases.

Batch shipping for small practices: We strongly recommend weekly case batches to optimize international shipping costs (small clinics recommended to ship once per week). Single-case trial orders are accepted, but ongoing single-unit shipments are not economically sustainable due to high international freight rates (we accept single-case trial orders, but do not support long-term single-case service because international shipping is expensive, single cases have no profit, we operate on thin margins and high volume).

Multi-location delivery (DSOs and group practices): We can ship directly to individual clinic addresses from a consolidated batch (we have experience serving chain clinics, know their pain point is packages need to go directly to each location). Provide location codes and shipping details at case submission.

Real Cases and Clinical Scenarios

The following scenarios are generic composites based on clinical patterns described in established, medically reviewed clinical guidance (see Cleveland Clinic and NHS in References) and common lab workflows documented in our internal quality database. They are not individual patient cases but represent typical clinical situations where hard night guards are prescribed and how lab workflow supports successful outcomes.

Scenario 1: Severe nocturnal bruxism with enamel wear

Clinical pattern (literature-based): Adult patient presents with documented enamel wear facets on posterior teeth, jaw muscle soreness upon waking, and audible grinding reported by a household member. Dentist prescribes a maxillary hard acrylic guard for tooth protection and muscle relaxation.

Lab process: Digital scans submitted with clear bite registration. Maxillary hard guard designed with adequate occlusal thickness (increase occlusal surface thickness, make occlusal surface uniformly flat to distribute occlusal force). Bite verified on articulator before fabrication (verify intraoral scan and bite record, mount on articulator for secondary verification). Guard fabricated via thermoforming, edges trimmed smooth, retention checked on stone model.

Takeaway: Adequate occlusal thickness and accurate bite records are essential. Insufficient thickness or incorrect vertical dimension can lead to premature wear-through or patient discomfort.

Scenario 2: Deep overbite with limited occlusal clearance

Clinical pattern (literature-based + KB): Patient with deep overbite presents limited interocclusal space. Dentist wants to fabricate a night guard but is concerned about appliance thickness.

Lab process: Case submission included note "occlusal space inadequate, 2 mm vertical clearance" (note occlusal space inadequate, write remaining space thickness, prompt for reduction amount or thin material selection). Lab contacted dentist before fabrication, confirmed selective grinding of opposing dentition had been completed, and proceeded with thin-profile design. Final appliance seated without interference.

Takeaway: Pre-production communication prevents remakes. When occlusal space is limited, note it explicitly on the prescription so the lab can plan material selection and design before starting fabrication (our technicians measure occlusal space on the model and inform the doctor; if the doctor needs it, we can also leave a note in the package reminding the doctor which tooth needs grinding and by how much).

Frequently Asked Questions

Q: Can I order a single case to test your lab's quality before committing to a partnership? A: Yes. We accept single-case trial orders (of course, and many new customers do exactly that). However, due to international shipping costs, ongoing single-case orders are not sustainable for either party. We recommend weekly batches once you're satisfied with initial quality.

Q: How do I verify your FDA registration? A: Visit the FDA Establishment Registration & Device Listing database, search for "Istar" or our establishment registration number (provided upon request). Our device listings cover custom dental prosthetics including occlusal guards (covers custom crowns, fixed bridges, inlays/onlays, veneers, custom implant abutments, implant superstructures, full/partial dentures, temporary restorations and other custom dental prosthetics). This is a public database maintained by the FDA.

Q: Do you provide a dedicated account manager? A: Yes. Every customer is assigned a manager and a customer service team (yes, every customer has a designated manager and customer service team). Technical questions typically receive responses within 1 hour during business hours (9:30 AM – 6:00 PM China Standard Time); outside these hours, response time may extend to 7–8 hours due to time zone differences, with a maximum of 12 hours.

Q: What if the patient's bite changes after the guard is delivered? A: Clinical adjustments (occlusal equilibration, selective grinding) are the dentist's responsibility and are standard practice for all occlusal appliances. If the original guard no longer fits due to significant tooth movement or a new prescription is needed, submit a new case with updated records. This is billed as a new case, not a remake (remake situation allows free redo if customer re-preps tooth; non-remake situation requires standard fee).

Q: Can you sign confidentiality agreements, quality agreements, or supplier contracts? A: Yes (all possible as long as customer needs it). We accommodate NDA, quality assurance agreements, and vendor contracts as needed (supplier intake questionnaires and annual audits also supported). Contact our sales team to initiate the process.

Q: What is your policy on supplying technical data sheets, biocompatibility certificates, or regulatory declarations? A: These documents are provided as part of our standard case package upon request. If you need them for internal compliance reviews, pre-market submissions, or payer documentation, specify this at case submission or contact your account manager (can provide English custom declarations, material certificates, product labels and related materials).

Q: How do I get a price quote and product catalog? A: Contact our sales manager directly (communicate directly with sales manager, and you can get it). We provide B2B lab fee schedules, material lists, and turnaround time grids tailored to your case volume and geographic location.
